Thanks for the callout. There’s a bug at the moment where Uniques from Belial’s Horder when selecting Harbringer of Hatred can drop with fewer base Affixes than intended. We’re planning to have this fixed in an upcoming patch update.
Until the fix, please select Duriel at Belial to avoid this issue…
